I fully understand that my IADA dismantler membership is contingent upon my having a current dismantler’s license. I further state the license number shown above is valid and will be renewed for the year of (Filled out the Year text box)  I further state that I agree to abide by the Association By-Laws, and Code of Ethics, which is included on the reverse side of this application.
